Physical and Dental Examination Requirements

All students are required to have a physical examination upon entry into school for kindergarten, in 6th grade and in 11th grade. All students are required to have a dental examination upon entry into school for kindergarten, in 3rd grade and in 7th grade. School dental screenings are offered in the Spring each school year. It is preferred that your family dentist completes this examination. Forms are available in the office or on the school website.

Hearing and Vision Screenings

Screenings are conducted by the Tuscarora Intermediate Unit in the Fall of each school year. If your child fails the screening he/she are re-tested by the school nurse. If they fail the secondary screening a referral is sent home and your child will require further evaluation by their primary care physician or eye doctor. The referral must be returned to the school nurse. If your child is prescribed glasses or a hearing device they should be worn to school every day.

Height, Weight and BMI Report

Pennsylvania School Health Code requires the recording of height, weight and body mass index for each school year. These results are available to view on the parent access site in Skyward. If you prefer a paper copy please let your school nurse know.

Kindergarten Requirements

• 4 doses of tetanus, diphtheria and acellular pertussis * (1 dose on or after 4th birthday)
• 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose)
• 2 doses of measles, mumps and rubella**
• 3 doses of hepatitis B
• 2 doses of varicella (chickenpox or evidence of immunity)
  * Usually given as DTaP or DTP or DT or TD
  ** Usually given as MMR
